Personal Information Consent Statement

 

本同意書說明國立中興大學(以下簡稱本校)將如何處理本表單所蒐集到的個人資料。
This Statement hereby sets forth how National Chung Hsing University (hereinafter referred to as “the University”) will process the personal information collected in this form.


當您勾選「我同意」並簽署本同意書時,表示您已閱讀、瞭解並同意接受本同意書之所有內容及其後修改變更規定。若您未滿二十歲,應於您的法定代理人閱讀、瞭解並同意本同意書之所有內容及其後修改變更規定後,方得使用本服務,但若您已接受本服務,視為您已取得法定代理人之同意,並遵守以下所有規範。
By ticking the “I agree” box and signing this Statement, you confirm that you have reviewed and understood, and agree to the terms of this Statement and its subsequent amendments. If you are under the age of 20, you may not use the service until your legal guardian has reviewed and understood, and agrees to the terms of this Statement and its subsequent amendments. Nevertheless, if you have used the service, you shall be deemed to have obtained your legal guardian’s consent and to agree to comply with the following terms.


一、基本資料之蒐集、更新及保管
I.    Collection, Updating, and Custody of Basic Information
1.本處蒐集您的個人資料在中華民國「個人資料保護法」與相關法令之規範下,依據本校【隱私權政策聲明】,蒐集、處理及利用您的個人資料。
This Office will collect, process, and use your personal information in accordance with the Personal Data Protection Act of the Republic of China and other relevant laws and regulations, as well as the University's Privacy Policy Declaration.
2.本處因執行業務所蒐集您的個人資料包括姓名、職員編號、聯絡方式(電話、E-Mail)等,請於申請時提供您本人正確、最新及完整的個人資料。若您提供錯誤、不實、過時或不完整或具誤導性的資料,您將損失相關權益。
Your personal information that the Office collects during the course of business includes your name, employee no., and contact information (telephone number and e-mail address). Please provide your correct, latest, and complete personal information at the time of application. Your rights and/or interests may be affected if you provide incorrect, false, outdated, incomplete, or misleading information.
3.您可依中華民國「個人資料保護法」,就您的個人資料行使以下權利:
You may exercise the following rights as regards your personal information in accordance with the Personal Data Protection Act of the Republic of China:
(1)請求查詢或閱覽。(2)製給複製本。(3)請求補充或更正。(4)請求停止蒐集、處理及利用。(5) 請求刪除。
(1) Request to inquire about or review your personal information. (2) Request copies of your personal information. (3) Request to supplement or correct your personal information. (4) Request to stop the collection, processing, or use of your personal information. (5) Request the deletion of your personal information.
但因本處執行職務或業務所必須者,本處得拒絕之。
However, the Office may refuse your request(s) where necessary to perform its duties or conduct its business.
若您因行使上述權利,而導致權益受損時,本處將不負相關賠償責任。
The Office shall not be liable for any related compensation for any loss of rights and/or interests resulting from your exercise of the aforementioned rights.


二、蒐集個人資料之目的
II.    Purposes of Collecting Personal Information
1.本處為執行國際事務處申請業務需蒐集您的個人資料。
The Office collects your personal information for handling application-related matters of the Office of International Affairs.
2.當您的個人資料使用方式與當初本處蒐集的目的不同時,我們會在使用前先徵求您的書面同意,您可以拒絕向本處提供個人資料,但您可能因此喪失您的權益。
The Office will seek your written consent prior to using your personal information for any purpose other than those for which it was initially collected. You may refuse to provide your personal information to the Office, but your rights and/or interests may be affected.
3.本處利用您的個人資料期間為即日起5年內,利用地區為台灣地區。
The Office may use your personal information in Taiwan for a period of five years commencing from the date hereof.


三、基本資料之保密
III.    Confidentiality of Basic Information
您的個人資料受到本校【隱私權政策聲明】之保護及規範。本處如違反「個人資料保護法」規定或因天災、事變或其他不可抗力所致者,致您的個人資料被竊取、洩漏、竄改、遭其他侵害者,本處將於查明後以電話、信函、電子郵件或網站公告等方法,擇適當方式通知您。
Your personal information is protected and regulated pursuant to the University’s Privacy Policy Declaration. If the Office violates the Personal Data Protection Act or if your personal information is stolen, leaked, tampered with, or otherwise infringed due to natural disasters, incidents, or other force majeure, the Office will, after verification, notify you by an appropriate means, such as telephone, mail, e-mail, or website announcement.


四、同意書之效力
IV.    Effect of Consent Statement
1.當您勾選「我同意」並簽署本同意書時,即表示您已閱讀、瞭解並同意本同意書之所有內容,您如違反下列條款時,本處得隨時終止對您所提供之所有權益或服務。
By ticking the “I agree” box and signing this Statement, you confirm that you have reviewed and understood, and agree to the terms of this Statement. If you violate any of the provisions set forth below, the Office may terminate, at any time, all rights or services provided to you.
2.本處保留隨時修改本同意書規範之權利,本處將於修改規範時,於本處網頁(站)公告修改之事實,不另作個別通知。如果您不同意修改的內容,請勿繼續接受本服務。否則將視為您已同意並接受本同意書該等增訂或修改內容之拘束。
The Office reserves the right to amend the provisions of this Statement at any time. The Office will announce any such amendments on its website and will not send a separate notification to you. Please do not continue using the service if you object to the amendments. Otherwise, you will be deemed to have agreed to be bound by any additions or amendments to the terms of this Statement.
3.您自本同意書取得的任何建議或資訊,無論是書面或口頭形式,除非本同意書條款有明確規定,均不構成本同意條款以外之任何保證。
Unless otherwise explicitly provided in this Statement, any suggestion or information obtained by you from this Statement, whether in writing or verbally, shall not constitute any warranty beyond the provisions of this Statement.


五、準據法與管轄法院  本同意書之解釋與適用,以及本同意書有關之爭議,均應依照中華民國法律予以處理,並以臺灣臺中地方法院 為管轄法院。
V.    Governing Law and Court of Jurisdiction The interpretation and application of this Statement, as well as any disputes arising herefrom, shall be governed by the laws of the Republic of China and the Taiwan Taichung District Court shall be the court of jurisdiction.
