1. Part of "My NCHU Era" Campaign.
2. Deposit 300 Ntd to The 3rd floor of the administration building counter OIA
(Please hand in the deposit within 3 days after filling in this form) otherwise will be considered invalid
3. One who has a motorcycle will be first considered (To distribute the food)
4. The quota of people is limited to 15 👤
5. Please bring your own 😷 mask.
6. Breakfast and Transportation are not provided, but we provide lunch.
